Team Trip Parental Release Form

I, as the parent or legal guardian of the participant listed below, (hereinafter referred to as “Participating Minor”) hereby acknowledge and agree that Participating Minor has applied to Saving Susan Ministry Inc. for permission to participate in Saving Susan’s short-term mission/ministry project to the location listed below, beginning on the dates listed below (said project is hereinafter referred to as “the Project”); and that Saving Susan has accepted Participating Minor on the Project subject to the terms set forth herein below.
