INDIVIDUAL RELEASE, HOLD HARMLESS AND INDEMNIFICATION FORM (SHORT TERM PROJECT)

I hereby acknowledge and agree that I have applied to Saving Susan Ministry Inc. (hereinafter referred to as “Saving Susan”) for permission to participate in Saving Susan’s short-term mission/ministry project to the location listed below, beginning on the dates listed below, for said mission/ministry (project is hereinafter referred to as “The Project”); and that Saving Susan has accepted me on The Project subject to the terms set forth herein below.
