I have applied to Saving Susan Ministry Inc. (hereinafter “Saving Susan”) to participate in a mission project to the location listed below beginning on the date listed below (said mission project hereinafter referred to as “The Project”). As a condition of Saving Susan Ministry allowing me to participate in The Project, I hereby acknowledge and agree to the following:
